Franklin Hills Country Club continues to work diligently to provide you with an affordable and best in class Health Care Program. Each of our insurance carriers and plan options remain the same for 2021!
Annual enrollment is your opportunity to learn about the 2021-2022 Health Care Program, review your current coverage and choose the best options for you and your family. Check out your applicable benefits highlighter for a quick side-by-side comparison of your benefits offerings for 2021-2022. To learn more, please review your benefits guide, watch the educational videos and review the additional resources.
Remember, this is the only time during the year that you can enroll in coverage or change your benefit elections, unless you have a qualifying event during the year. You have 30 days from the qualifying event date to notify HR to make the changes. Common life events are: marriage, divorce, birth, adoption, and loss in coverage.
How To Enroll
- You must complete an election form, even if you are waiving coverage.
- Complete the No Change Election Form if you are not making any changes to your current elecitons.
- Complete the Full Election Form if you need to make a change to your current elections and/or enroll in coverage for the first time.
- Completed election forms must be returned to Rosanne, no later than Monday, May 17, 2021 at 5 p.m.
- You will not be allowed to make changes after the open enrollment window closes, unless you experience a qualifying life event.
